Our licensed clinicians, specializing in primary care and geriatric medicine, adopt a holistic approach to managing chronic conditions for homebound patients. Physicians
The Physicians at MD at Home provide Advanced Primary Care services to patients who have been confined to their home due to age or disease. We work with chronically ill patients to manage their chronic conditions with a multi-disciplinary, home-based, team approach. Our Physicians gain a sense of fulfillment and satisfaction during their practice due to the unique way they interact with patients.
We are looking for Physicians with the following qualifications and qualities:
* Passion, Leader, Inquisitive, Enthusiast, and Strong Communicator
* M.D. or D.O. degree from an accredited medical college or university
* Licensed as a Physician in the state of Illinois, Indiana, or Florida
* Valid state Controlled Substance License or the ability to obtain a license prior to starting employment
* Valid license with the Federal DEA or eligible to obtain a license prior to starting employment.
The Home-Based Primary Care model continues to allow Physicians the ability to spend more time with the patient and maintain work-life balance. Medicare’s Independence at Home Demonstration Project has shown Home-Based Primary Care visits to be an innovative and effective patient-centered care model which improves patient outcomes.
Physician Assistant
MD at Home is a growing Home-Based Primary Care practice. We work with chronically ill patients to manage their chronic conditions with a multi-disciplinary, home-based, team approach. Our Physician Assistant gain a sense of independence, fulfillment, and satisfaction during their practice due to the unique way they interact with patients.
We are looking for Physician Assistants with the following qualifications and qualities:
*Passion, Leader, Inquisitive, Enthusiast, and Strong Communicator
*Degree from accredited Physician Assistant program
*Licensed as a Physician Assistant in the state of Illinois, Indiana, or Florida
*Valid state Controlled Substance License
*Valid Controlled Substance License with the Federal DEA

Nurse Practitioner
At MD at Home, our Nurse Practitioners (NPs) play a crucial role in providing Advanced Primary Care services to homebound chronically ill patients. With a comprehensive and multi-disciplinary approach, our team aims to make a real difference in the lives of our patients.

Fellowship Program:
If you are a recent Nurse Practitioner or Physician Assistant graduate, MD at Home Advanced Primary Care fellowship program is designed to ensure your success. With clinical support from experienced doctors and a mentor, you’ll receive valuable guidance, and consistent feedback. The program includes Up-to-Date access, presentations, blogs, and online lectures powered by the Gerontological Advanced Practice Nurses Association (GAPNA).
Qualifications:
*Completion of an accredited PA or NP master’s program
*Pass the PA or NP National board examination
*Obtain state licensure as a PA or NP
*Obtain DEA licensure within first month of the fellowship
*Successfully pass an employment background check and drug screening

Podiatrist
The Podiatrist at MD at Home provides Podiatric care services to patients who have been confined to their home due to age or disease. Our Podiatrists gain a sense of fulfillment and satisfaction during their practice due to the unique way they interact with patients.

We are looking for Podiatrists with the following qualifications:
* DPM degree from an accredited medical college or university
* Licensed as a Podiatrist in the state of Illinois, Indiana, or Florida
* Valid state Controlled Substance License or the ability to obtain a license prior to starting employment

Social Worker
The Social Worker at MD at Home will support the psycho-social elements of our evidence based, data driven care plans in the greater Chicagoland area. We work as a part of a multi-disciplinary home-based care team in the community to manage chronically-ill, low-income patients.
Responsibilities of the role:
*Analyzes the patient population’s aggregate data and develops actionable care plans addressing the major social determinants of health and psycho-social challenges of the population
*Performs comprehensive psycho-social evaluation of individual patients to assess economic, social, and emotional needs of the patient and links the individual patient to the overall population plan
*Works closely with members of health care team to develop a care plan based on assessment.
*Identifies and assists in resolution of the patient population’s needs related to
- ADL’s, including but not limited to hygiene, food access, housing, and mobility
- Social including but not limited to isolation, spirituality, and safety
*Develops and tracks a scalable, multi-disciplinary, multi-organization plan to address the patient populations needs
*Maintain clear and concise documentation.
*Communicate and collaborate with a multi-disciplinary care team
A successful candidate has the following qualities:
